Pretrial Intervention (PTI) Forms –

          file the appropriate PTI Proposed Stipulated Order [In or Out of Custody]
         (filled out entirely with special conditions and reporting date included)
           submit to the assigned judge when agreed upon, and
           provide the PTI reporting Instructions to the Defendant. 
There is no need to docket the case and request a hearing date.
