Forms

Initial Report Forms (due to be filed with the court within 60 days of the date Letters of Guardianship are issued)

Initial Plan for Minor This form should be used if you are guardian of the person, and the ward is under the age of 18.

Initial Plan for Adult This form should be used if you are guardian of the person, and the ward is over the age of 18.

Verified Inventory This form should be used if you are guardian of the property (adult or minor).

Annual Report Forms (due to be filed with the court within 90 days of the end of the reporting period)

Annual Plan for Minor  This form should be used if you are guardian of the person, and the ward is under the age of 18.

Annual Plan for Adult This form should be used if you are guardian of the person, and the ward is over the age of 18.

Physician Statement for Annual Plan This form can be used to fulfill the requirement for the physician statement for the annual plan of the person for an adult.

Annual Accounting This form should be used if you are guardian of the property (adult or minor).

Simplified Accounting (Adult or Minor)
This form is to be used ONLY when all of the ward’s assets are in a frozen/restricted account, and the only transactions that occur in that account are interest accrual, deposits from a settlement, or financial institution service charges.

SIMPLIFIED ANNUAL PLAN

If you have received a court order that authorizes you to use a Simplified Plan form, you must use the following, current version of the forms.  You may use this form ONLY if the court has authorized you to do so.

  • For Guardian Advocate (appointed for a developmentally disabled person under Florida Statutes Ch. 393)  [PDF]

CERTIFICATE OF SERVICE

Use the Certificate of Service form, when showing service of a report on the Ward, Attorney for Ward, or other interested party.

PROFESSIONAL GUARDIAN FEE STATEMENT

Professional Guardians should use this form, in connection with their guardian fee petitions.
